Perception vs. Reality

Mike going to visit his grandmother
This was one of her good days. My mother had warned me that she might not know who I am at first.
"Ellie on your lap..."
My mother's name is Ellen, but everyone always calls her Ellie...Then I remembered-my grandfather's name was Michael
I wanted to say,"Nana, this is Mike your grandson, not Mike your husband."
This quote is referring to when Mike first gets to the nursing home and sees his grandfather and at first he thinks that she might not remember him but she says his name so he gets hope and thinks that she remembers him.
Here Mike is starting to really notice that she thinks he's somebody else and he realizes that she thinks he's her husband because they share the same name and she talks about Ellie which is his mother's name.
Mike is trying to explain the reality of the situation by telling her that she's talking about the wrong Mike but he doesn't actually say anything but instead he goes along with it for the most part.
