The Narrator treats the Old Man with lots of care during the week before he killed him. The Narrator was a madman, with his motive being what he called the Old Man's "Evil Eye".
The heart beating causes the Narrator's ears to ring and it makes him go nuts. He knows the Old Man is horrified, and the heart beating gets louder and faster and the Narrator gets more angry.
Every night, up until the eigth, the Narrator watched the Old Man sleep through the night.
The Narrator snaps and pulls the Old Man out of the bed. The Old Man then screams out of terror before the Narrator then precedes to suffocate the Old Man with the bed covers. Once the Old Man dies, the Narrator prepares for the next part of his plan.
On the eigth night, the Narrator was preparing to murder the Old Man, when all of a sudden, he snickered at his plan, waking the Old Man.
After waiting an entire hour, the Narrator cautiously entered the room with his lamp, shining just a very thin ray of light onto what he considered the "Evil Eye". A few minutes later, he heard a beating noise and then realized, it was the Old Man's heart.
