  • Name: John Smith DOB: 21/04/1928 Allergies: NKA Primary contact: Jessica Smith Adm Hx: Mr. Smith, 89M, found in apartment living room when neighbour called 911 after he had not been seen in 3 days. Mr. Smith found unconscious, covered in urine. Mr. Smith lives alone, and neighbour states family has not been to visit in 2 years. Neighbour states Mr. Smith occasionally complained of blurred vision and headache. Test Results: CT scan: ruptured saccular aneurysm in ACA. Minor subarachnoid hemorrhage. Progress Notes: VS: BP 103/62, HR: 102, T: 36.4, RR: 11, O2Sat: 90%. Neuro: Pupils sluggish. Pt not oriented to time or place, fatigued, awake in intervals. GCS: 8.  Daughter Jessica called and informed, will be arriving shortly. Blood work results complete.

  • Is it true I don't have to pay for my dad’s medications if he stays at the hospital?  How long can he stay here?              Can he still live alone after             discharge?

  • Three WHOLE days on the floor?? You  told us you were checking up on him every day and that he was "doing well!!" You're even getting PAID to care for him!!
  • WHAT?!?!? You have NO right to get angry at me!! I do my part, and it's definitely more than you do. You live close by too!!! How DARE YOU think YOU can blame me for all this!!!!
