Hamlet speaks to the ghost of his father where the ghost tells him who killed him and to get revenge but leave his mother alone.

Ophelia goes to her father like she is supposed to do and tells him of the things Hamlet has been sending her. He reads a letter he sent to her and learns of Hamlet's "madness."
"He raised a sigh so piteous and profound as it did seem to shatter all his bulk and end his being"
Hamlet kills Polonius after hearing him yell from behind the tapestry after the play he conducted.
"How now, a rat? Dead for a ducat, Dead!"
After the death of her father, Ophelia drowns in a brook while Laertes and Claudius come up with a plan to kill Hamlet.
The plan doesn't go as planned which causes Claudius, Gertrude, Hamlet, and Laertes to die with Hamlet making young Fortinbras King of Denmark.
