- I'm going to send you over to the lab for a current HbA1c test. Do you have a current Opthamologist? If yes, I'd like you to follow-up with him or her on your Retinopathy. Before you leave today, please schedule a follow-up visit, as we want to closely monitor your Nephropathy.
- Tom is 67 year old male with Type II Diabetes. Tom is visiting Dr. Jones for his annual wellness visit.
- Dr. Jones receives a quality alert via the QRA Point of Care solution, advising her of the quality measure impacted and recommended actions.
- Dr. Jones sees the advice and discusses the follow-up with Tom.
