- Mthoko has not been well for the past few days. He feels hot. He is also coughing and has a runny nose. Before bed I noticed a rash on his face. This morning the rash is now on his whole face, chest and arms! I’m worried, my love.

- Hi Sr Khumalo, Mthoko is running a fever, has a rash and started vomiting this morning. He has not been eating well since yesterday afternoon and this morning when I was bathing him I noticed the rash that I had seen on his face last night was now on his body. His eyes are watery and nose is runny as well.
- Hello, I’m Sr Khumalo. I’ll be assisting you. May I start checking Mthoko’s vital signs while you tell me why you brought him in today? May I please have his Road To Health Card?
- Mthoko’s temperature is very high (38,5 degrees) and he appears to be dehydrated, he needs to see Doctor Mbatha so she can order the relevant tests.
- A fourth case within a period of 24 hours? Please put the child in an isolation ward, call the lab to do the PCR and bloods. Start him on IV fluids and give Panado 180mg 6 hourly for the fever while we wait for the results. Do we have the results from the other cases?
- Dr Mbatha, we just received a call from OPD, Sr Khumalo says she is admitting another patient. The child presented with similar symptoms.
- The results are in Doc. We tested both serum and swabs from the suspected patients. In the serum specimen we tested for the presence of antibodies against measles while in the swab we conducted PCR test for presence of the measles RNA. Positive results from both these samples confirmed infection with the measles virus because IgM antibodies were detected indicating an acute infection. The PCR test was done to rule out the possibility of positive results due to vaccination and due to its more sensitive nature. Genotyping analysis confirmed that all the infected individuals are infected with one strain, and that the source is not from vaccination. We need to report the cases to NICD immediately, please remember to send a copy to the focal person at the NMC sub-district.
- Measles is airborne and can be contracted when individuals come into contact with droplets expelled by infected individuals when they sneeze or cough. We need to ensure that the infected individuals are isolated, practice social distancing and hand hygiene. Contacts that are susceptible need to be given post-exposure prophylaxis to curb the spread of the disease to prevent straining our healthcare facilities.
- Dr Mbatha {completing the notifiable disease forms for 4 active Measles cases with detectable IgM antibodies} N.B (1 Case has detectable IgG antibodies and the other cases have none)Form emailed to the NICD (national), NMC focal person (Sub-district/District), Hospital CEO. The Minister of Health is made aware.

- Mayor we have an outbreak of measles. There are 4 cases that we are currently aware of, they all presented to the Springville OPD. No IgG antibodies were detected in 3 of the cases, indicating that they were not vaccinated. The children presented with similar symptoms and they are all in the infectious stage. We have sent notification forms to the NICD. The children are in an isolation ward. We are tracing possible contacts.
- I need to communicate with the NICD and see how far they are with the task team. We need to make sure all the contacts are found.
- We will need to make the community aware. They need to know what symptoms to be on the lookout for and what actions to take.They need to be assured that we have this under control.
