Sofia goes on to tell Elena about her toxic relationship with a guy in the grade above that she has been with for 6 months so far. He is controlling and makes her feel worthless. She has silently been dealing with his hurtful nature this whole time. She still loves him through the pain, but she justifies his actions by saying "he is going through a rough time" and "he will change." But deep down, she knows she deserves so much better.
Elena feels horrible for Sofia, but she doesn't know if she should just comfort her cousin or be straight up with her and say something like: “Sofia, you can’t keep letting him treat you like this... you need to break up with him, as soon as possible. He’s not good for you, and you know that.” Elena is conflicted because knows how much Sofia loves this guy and wants to protect him, but she cannot bear seeing her cousin repetitively get hurt.

Sofia tells Elena that deep down, she knows that the only way to stop this hurt would be to break up with her boyfriend, but it's so hard for her to because they've had so many good memories together and she loves him too much to "hurt" him like that. She also mentions that she's hesitant about this decision because she doesn't want her boyfriend to react bad towards her. So, Sofia is stuck between the moral decision to save herself from hurt or save her toxic boyfriend from hurt.
Elena finally builds up the guts to harshly tell Sofia that she needs to break up with him, because Sofia would not listen to Elena's advice if she were being rather friendly about the situation. Elena also just could not bear seeing her own cousin in such a tough situation, so she couldn't just sit there and do nothing for her. So, Elena decided that she would rather be harsh and get the point across to Sofia, even if that meant that Sofia would be unhappy with Elena for a bit, because in the long run, Sofia will be thankful that Elena smacked some sense into her.
So in the end, Elena made the right decision to tell Sofia to save herself from repetitive hurt, even though it meant slightly hurting Sofia; and Sofia stuck up for herself and made the right decision in breaking up with her toxic boyfriend.
