  • Your General Practitioner and the research team invite you to be involved in a study to improve your heart health
  • You will contact the research team to answer any queries you may have, and provide"Informed Consent."  An appointment will be made to have a heart health check at your GP clinic.
  • You will come to your GP clinic for an appointment with a research nurse who will collect demographic and heart health information. Including cholesterol, blood pressure, blood glucose, height, weight and other information.
  • You will be randomly allocated to the Control Group or the Intervention Group. Both groups will complete assessments at Baseline, 6 Months and 12 Months
  • If you are allocated to the Control Group
  • You will be provided with - heart health information - a recommendation to follow up this appointment with your GP - monthly contact with the study team - you will come in next for the 6 month assessment
  • You will have a longer appointment with the research nurse to discuss your heart health results, and ways to improve these
  • If you are allocated to the Intervention Group
  • You will be shown your risk of having a major heart health problem. Key risk factors will be discussed.
  • There will be some options that your GP can discuss with you to improve your heart health
  • And there are some things that you can do to help your heart that we will spend some time discussing
  • After the conversation you will take home a list of the things you have listed try to improve your heart health.
  • Following this you will have an appointment with your GP to review your medications and discuss any medical issues you may have
  • After you go home we will be in touch to provide assistance and motivation in helping your heart. This is likely to be every two weeks via SMS and once a month via phone.
  • If you are in the Intervention Group
  • After you go home we will be in touch monthly via phone to answer any queries and keep in contact.
  • If you are in the Control Group
  • Heart health can be improved and this will be discussed with you
  • At 6 Months and 12 Months after you became involved in the study we will conduct heart health checks again and will ask you to answer some questions about your health and how you are feeling.
  • For all participants.
  • Assertive Care is a person focused approach to treatment by having clinicians proactively collaborate with patients and has been useful in physical and mental health.
  • People with severe mental illness often have a high risk of cardiovascular disease and poor heart health.
  • We have developed this study to use Assertive Care with people who have severe mental illness to improve their heart health outcomes. We would like your feedback on some key parts of the project.
