- Wow, I can not believe we have been working with you and Sophia for six months now. Can you tell me how the IFSP is working out? What is Sophia's communication like now? Is it better, worse, the same? Would you like to include anything else in the IFSP plan to help better her language development?

- I think myself, our nanny, and our speech therapist would all agree that Sophia is progressing as she should in her speech and communication. We all think at this point that Sophia is ready for more outcomes to be added to her IFSP. I know I would like to see her use more words, use 3 to 4 word phrases or sentences, and answer simple questions.
- Today is our annual review of your IFSP. We will discuss the progress Sophia is making in meeting her measurable outcomes in your IFSP and if any changes need to be made. With that being said, would you say that Sophia is meeting the measurable outcomes and is progressing even more in her speech and communication development, than from when we last met?

- Since Sophia is almost 3 years-old, it is time to transition her into Part B of IDEA. What this means is that it is time to transition Sophia into preschool, as she is eligible for preschool services since she still does not say full sentences and she has difficulty interacting with peers and adults. I will work together with the preschool program and be the lead in this transition to ensure it is smooth for you and Sophia. The school system will evaluate Sophia and create an IEP if they find that Sophia has a disability and will need special education services during her years at school.
- I can not believe my Sophia is going into preschool. Thank you for everything and for all your support. I really hope this transition will be as smooth as you say you will make it. I hope the school makes the IEP process as simple as you made the IFSP process for us over this past year. I can not thank you enough for you and your team's hard work in helping Sophia improve her speech and communication development.

- We think her communication has definitely increased. She now listens when we tell her to do something and she is using more words than before. She even tells us with her words when she wants more juice or a snack. The speech therapist who we work with thinks her speech and communication is improving every time we see her, which is great to hear. I think our current IFSP plan is working perfectly for Sophia's development and does not need to be changed at this time.
