  • Good day nurse! I have had this series of headaches for about a week now. I want to have an appointment with a doctor. I haven’t been able to sleep properly because of this pain. There are times where I have shortness of breath too.
  • Identification Phase: This is where the selection of appropriate professional assistance is done. This is also the phase where the patient begins a sense of belonging. This is where the nurse also takes leadership and takes charge of the patient.
  • I am Kath, your nurse and I will be taking care of you. When did you start feeling these symptoms? I’ll take your vital signs then do some tests. I’ll consult a doctor and she will see you shortly.
