- Good day nurse! I have had this series of headaches for about a week now. I want to have an appointment with a doctor. I haven’t been able to sleep properly because of this pain. There are times where I have shortness of breath too.
- Identification Phase: This is where the selection of appropriate professional assistance is done. This is also the phase where the patient begins a sense of belonging. This is where the nurse also takes leadership and takes charge of the patient.
- I am Kath, your nurse and I will be taking care of you. When did you start feeling these symptoms? I’ll take your vital signs then do some tests. I’ll consult a doctor and she will see you shortly.
- I started feeling these symptoms for almost two weeks now. I have been burdened by this headache, and I feel very fatigued. I also get occasional chest pains and difficulty in breathing.

- This is where the nurse become a teacher and a resource person. The nurse becomes a teacher since she gave additional knowledge to the patient and she became a resource person because she provided needed information for the patient to be able to understand deeper.
- I did not expect to be hypertensive because my parents don't have them, and I am well aware that hypertension is a genetic disorder. Maybe my grandparents have them.
- What I know is that hypertension does not only happen in older adults but it could also happen to younger people. Hypertension is also the leading cause of stroke.
- Oh, what do you know about hypertension? It seems that you have some knowledge about it? Also, you are right about hypertension being a genetic disorder.
- Yes, Hypertension can happen to all ages and it is also recommended to monitor a healthy diet and engage in physical activities together with the prescribed medicine that we gave you for you to avoid obesity and will later on develop stroke.
- Resolution phase: The patient no longer needs professional services and gives up dependent behavior. The patient’s needs have already been met by the collaborative effect of patient and nurse. The patient then drifts away from the nurse’s bond, and a healthier emotional balance is demonstrated, and both become mature individuals.
- Thank you for your assistance. Now that I have learned more information about hypertension, I will make sure to keep monitoring my health at all times and follow the advice given to me.
