I have communicated to everyone that I have isolated and locked this out. I can now leave safely knowing everyone else is safe.
I know I locked it off.. Someone has cut it off! Do you know anything about it?
I cut it off because I let someone else work on it to get the job done faster..
This site could be live and everyone would be at risk. We need to ensure that it is safe to work before resuming..
The FSE arrived at site safely, attended a site induction where appropriate H&S regulations were covered, including LOTO. He is now beginning the job after the toolbox talk.
The site is isolated and safe, everyone should get back to work.
It is coming to the end of the day..time to Lock Out.

Everyone returns to site to find that the locks have been cut off. This site is now unsafe. This is a high risk environment that could even result in death.
The FSE is disappointed with the lack of responsibility and care due to the customer ignoring the Zero Harm culture. He tries to have a civilised conversation about the risks and why the breach was made.
The customer insists on the FSE getting back to work immediately; ignoring the risks and the FSE's opinion. The FSE does the right thing and politely refuses to work further until the site is safe again.
The FSE is recognised and awarded for his attitude in an unpleasant situation. No one should put themselves or others at risk just because they are under management pressure.
