The gravedigger found a skull and Hamlet recognized who it was and tells Horatio that he knew the guy and loved him dearly when he was a child.
Laertes speaks of Ophelia dying and jumps in her grave wanting to hug her one last time when Hamlet reveals himself saying he loved Ohelia like no one ever has.
The "pearl" which was the poison in the wine get messed up and the Queen accidently drinks it and dies then Hamlet makes the king drink it and dies. Then Laeres gets poisoned not intentionally by his own sword blade and dies. Finally Hamlet drinks the last of the poison and also dies.
