Dr. Watson visits Sherlock's appartment only to be told he is very sick and hasn't had any food or drink in three days.
Sherlock Explains to Watson he believes that the culprit is an eastern disease and tells Watson to find a man named Culverton Smith instead of a doctor to help him.
Watson also notices a box sitting on the floor and sherlock shouts telling him not to touch it.
Watson goes to meet the man mentiond by Holmes who is supposed to have some knowledge of the disease. They then talk about their respect for Sherlock in his line of work.
Sherlock and Smith begin with a mostly one sided conversation when Smith gives Sherlock a drink of water. Sherlock then tells Smith that he knows Smith killed a man they both knew and that he had proof now. Smith then reveals he was trying to kill Sherlock and sent the box as a way of collectin blood and infecting Sherlock with a disease
Sherlock then reveals he had never actually been sick, had seen through Smith's plan and was just faking it and that the drink of water was all the energy he needed to get up and confront Smith.
It ends with the authorities arriving due to clever planning on Holmes' part and Smith is arrested for murder.
