After Hamlet's soliloquy, he goes to visit his mother in her room to try and talk some "sense" in to her, and gets in a big fight with her about her morals, and in the events of the fight, he ends up "accidentally" killing Polonius. This is where Act IV starts.
He's crazy and it's scaring me, I don't know what to do!
I've had enough of this, we'll send him off to England for punishment.
Claudius sends Rosencrantz and Guildenstern out to find Hamlet and Polonius's body. When they speak to Hamlet he refuses to talk about the body, and is then confronted by the king, who proceeds to have Hamlet shipped out to England, where the mad man comes across an army, led by Fortinbras.
We are just passing through, headed to England.
I am leading the army of Norway to an attack on Denmark, who goes there?!
In Act IV, Hamlet discovers a note on his way to England that orders his death, so he bails, heads back to Denmark, and leaves Rosencrantz and Guildenstern on their way to England, except he changed to note to order their deaths. Back in Denmark, Laertes and Claudius scheme against Hamlet, especially after receiving horrific news of Ophelia.
Ophelia has drowned!
L: Shall we poison him? C: No, instead you will duel him in a swordfight!
A message sir: Hamlet is on his way back to challenge you.
Hamlet returns, agrees to fight Laertes, however, he didn't know the swords would be tipped in poison, or that there was a backup plan/poised drink that a certain someone(Gertrude) would accidentally consume. The following transpires: (DEATH)
