One source of contamination that is happening is animal waste, it percolates into the ground from farm feedlots causing contamination from continuous animal waste going to the ground water.
Another problem being underground storage tanks developing a leak, which commonly occurs as a tank ages, its contents can migrate through the soil and reaches the ground water.
A way to fix this would be by making a better effort on making sure there are no leaks and checking up to make sure tanks are not too old to where they leak. And for the second example to keep animal waste in an area where it is less likely to effect the groundwater underneath.
