Storyboard Text

  • After hearing about the new program from a neighbor, the Gonzalez family meets with an ambassador that belongs to their church. They chat after services.
  • I joined the program last year and like it because you get to prioritize the goals you and your family want to work on and choose your own mix of services to help you accomplish them.
  • Great! I can help you fill out some basic info and make a first appointment. This welcome packet also has more info.
  • That sounds pretty neat. We'd like to try it out.
  • The next step is to choose a Well-Being Coach. They will be your guide throughout the process. You can schedule an appointment with them in the next week or two at a location of your choosing. They will even come to your home!
  • Here are some local Well-Being Coaches that match your family's goals and needs.
  • Karen J.
  • Saul K.
  • Schedule an appointment with Tori?
  • Tori D.
  • It sounds like managing Daniel's asthma has been challenging.
  • Lifestyle Survey
  • During the meetings, they discuss their goals and things that are important to their family's health and wellness. Their Coach helps them prioritize what goals they might want to work on first.
  • Yes... especially since my ex-husband still smokes in the house when he takes Daniel for the weekend.
  • We have also been worried about bullying at Kimberly's school. We feel stressed and overwhelmed.
  • Before I leave, I can help set up a family care appointment with a dedicated care team. Alicia's team might be a good fit for your family. I'll make sure an asthma specialist is there too. Through the portal, you can share any information you think is important for them to know.
  • Thanks for sharing that. I'm adding some notes into your family profile. You can look through these at any point and feel free to add to them.
