Mr. Smith is being discharged home from the hospital after a hypertensive crisis. He has been prescribed 3 new blood pressure medications. Home health care will set up a mobile health blood pressure monitor in their home tomorrow morning that will send the data to his cardiologist via wi-fi internet. He is instructed to take his blood pressure twice a day in the morning and in the evening.
After 2 days, Mr. Smith's blood pressure at 8 am was 175/105 and has been consistent with this trend since being home. The nurse monitoring Mr. Smith arranges a telehealth visit remotely with the cardiologist. Mr. Smith complains of a slight headache and is instructed to come to the office later that morning at 11 am.
The cardiologist increases the dosage on his hydralazine and his amlodipine. Mr. Smith checks his blood pressure that evening after the dosage change and it has improved to 150/89 at 8 pm. The next morning, Mr. Smith's blood pressure has continued to improve to 140/78.
