Before Hazel met Augustus, she was doing well with her illness. She still was more antisocial and went to the support group more frequently than she did after her and Gus started to hang out.

At first when Hazel and Augustus started to become friends, she was still afraid to open up to him and to become close because she knew that they both had cancer and death was inevitable and that if she was to become too close the day one of them passed away would be incredibly heartbreaking and was iffy on if she should becomes good friends or not but later decided to.
At the end, Augustus' cancer spreads from his leg to his lungs. Throughout their friendship, they have had many adventurous journeys. They have flown all the way to Amsterdam to meet an author of their favorite book to ask some questions about what happened at the end. Cancer was a huge problem for them to do more things because they were limited to only doing some things and not as many things as an average teenager that doesn't have cancer can do.
The same way was for Augustus. Before he started to attend the support group frequently and met Hazel, he did his regular check ups, was making miraculous progress and continued playing sports and video games in the free time he had.
Augustus was open to making new friends when he met Hazel. He knew if he was to become close to her, and something was to happen to her, he would be torn to pieces, but didn´t care. He wanted to live his teenage life and have fun with Hazel whether or not they both had cancer or were perfectly healthy human beings.
After Augustus found out he had a limited amount of time before his cancer would spread to his lungs, he asked for Hazel and Isaac to both meet him at the church where they had their support group meeting once a week. He also asked for them both to say their goodbye because nobody knows when someones last day is going to be and for them to say what they want before he left this Earth.
