  • 1. Have records that need to be scanned go through the prep of all required documents before it is scanned. The prep staff should have an audit form that they sign and date when finished and leave clipped on top of the documents. During the prep the staff should check for correct patient name, patient number, document name, page numbers are in order, remove staples, etc.

  • 2. The scanning specialist should scan and name the documents according to the facility standards, example: patient number + facesheet or patient number+ H&P. The scanning specialist should then sign and date the audit form to signify it was scanned.
  • Backlog
  • 3. The indexing specialist then does a quality check on the documents to make sure they are scanned correctly. If it is done correctly it can then be attached to the correct patient file. If not the document should be sent to the scanning specialist to be rescanned.
  • Setting a Productivity standard
  • 4. Throughout the day each staff member in this process should log how many files or documents or pages they audit, scan, or do a quality check on so that their productivity can be assessed. Each rescanned document is not added to the scanning specialist’s log.

  • 5. To deal with the backlog of documents needing rescanning I would propose that one day a week is devoted to rescanning the backlogged documents until it is finished. These documents should also go through the same process as the other documents to ensure it is done correctly.
  • 6. To create the productivity standard I would average the documents for each part of the process; auditing, scanning, quality control. I would then set the standard for each position as the middle point between the average and the highest number for each position. Example: auditing average is 80 documents and the most documents audited is 100. The standard is 90 documents per day.
