Wanneer er iets misgaat, vooral in een grote organisatie, kan er een instinct zijn om vingers te wijzen en de schuld te verschuiven. Dit is waarschijnlijk niet de beste manier om te voorkomen dat het probleem zich in de toekomst voordoet. Sakichi Toyoda, de oprichter van Toyota Motor Company, ontwikkelde het '5 Whys' -proces, een soort oorzaakanalyse, om problemen die zich binnen het bedrijf voordeden te identificeren en oplossen. Het is sindsdien algemeen aangenomen als onderdeel van lean manufacturing, Kaizen en Six Sigma methodologie.
