You have been in situations where drug businesses have taken over all the top of Midwestern states and the economy is severely impacted; to the point where some businesses were suddenly closed due to lack of large amount of hospitalization and minor deaths from an illegal substance that came from foreign countries. It's time that you investigate and fight back what ever comes against you.

- Lorenzo Cardinale is called by his adviser Natalia Kursk for an important meeting about how everyone plans to terminate drug businesses that have drove other places out of business in Lansing Michigan. Don Gastone
- Infiltrated Drug Import
- Lorenzo meets his cousin Tyler Cardinale who in in the United States Navy. Tyler talks to Lorenzo about each others early life then speak about the Spanish Lords and the Yardies taking over Northern Illinois and Northern Michigan
- Don Meetings About Drug Trafficking.
- Lorenzo's boss Edmondo Camarda takes Lorenzo to the airport to tail a suspected contract killer Bryon Sauveterre to assassinate Edmondo for bringing down the restaurant business but those were false rumors. The truth will be be revealed in the next mission.
- Investigation Continues
- The most dangerous drug, Ketamine, and herorin sneaks its way into Indiana and eventually Illinois. Some street gangs make money off of selling illegal goods. This negatively affects the Italian mafia because, some of their customers are facing severe health and financial problems and the police aren't doing much help. Some made men are ordered to poke around to find the roots of the problem and diffuse them.
- 7 Dons were meeting all in the same room together with their underboss discussing how will they deal with drug trafficking, where is it going to, who is getting paid the most, and who is primarily responsible. They take action by first doing a business watch, making sure no one is doing drugs on their turf. Two drug dealers were caught and questioned. One revealed it will being shipped by someone from Indiana.
- Inside a mafia family members home with Lorenzo, they discuss how to find the source where drugs are coming from. Two volunteers to pretend to be a customer. After searching for some information, they slowly make progress. They spy on a drug dealer then get ambushed. After gather some intel, they take it to their dons, determining what should they do next.
