- He attends his local District General Hospital Emergency Department as soon as he can to get assessed for his symptoms

- He books himself into the department at reception. After providing his details, the receptionist is able to find Sid's medical history and medications from his local GP practice. These are imported into the Hospital EHR, and Sid is booked in to be seen.
- At Triage, the nurse takes Sids observations, blood tests and does an ECG. They also ask Sid a few questions about why he came in. They note Sid's blood sugar is very high.Sid mentions he has had his blood sugar tested in the past but is not sure what the result of this was.
- The nurse inputs Sid's observations and triage history into the hospital EHR. They also are able to connect to the West London Health Network to find out Sid's last blood tests and glucose level, which have always been normal. Based on this, and the the high NEWS score that flags on the system, the nurse puts Sid as a priority to be seen
- The Emergency Department doctor assesses Sid. She is concerned about the symptoms, and organizes a head scan. As the raised blood sugar is a new finding, she also sends off additional tests to investigate this.
