In the year 2180, overpopulation and a lack of vital resources like food, water, and shelter caused billions of people to die. The United States was hit the hardest, with only the capital city left standing when the dust settled.
Outside of the city, ruins of old buildings and a desolate wasteland were the only things that could be found. The landscape was uninhabitable. The remaining citizens were forced to move to the capital city in order to survive.

During the chaos, a dictator rose to power named Josh Lee. He had total control of society and used modern technology to rigidly control the population and the city. He also created strict laws that set the population to a fixed number that couldn't be exceeded.
Whenever a baby was born, the doctor would kill the mom in order to offset the increase in the population. Out of desperation, many soon-to-be parents would murder a random stranger on the street so that the mom wouldn't be sacrificed.
For Tim, he could never bring himself to kill a random stranger. But after his wife went into labor, that all changed.
The nurse got the couple a hospital room, and the doctor came into the room to help Tia, Tim's wife, deliver the twin babies.
