Visualizing goals is just one step in achieving them. Students can choose to use the goal they've created a storyboard for or set another goal for themselves. Following the SMART acronym, Specific, Measurable, Actionable, Realistic, and Time, students will make a goal for themselves for the year. In a month or in a few months (at your discretion), have students refer back to the worksheet. Are they still on the path toward their goal? What things might they have to adjust in order to keep going? Students may make tweaks as necessary, understanding that sometimes things don't go according to plan, but there's always a way to keep moving forward!
Clicking "Use This Assignment" will copy the worksheet into your teacher account. You can customize any part of it if you'd like to further tailor it to your students, or use as is. Once you're done editing, just save and print the worksheet!

Guide students to reflect on their progress by facilitating a class discussion after they’ve worked with the SMART Goals worksheet. Sharing insights helps students stay accountable and learn from each other’s experiences.
Explain the purpose of the discussion and remind students that all reflections are valued. This creates a safe space where everyone feels comfortable contributing.
Ask questions like, “What’s been your biggest challenge?” or “How did you measure your progress?” Open-ended prompts encourage deeper thinking and honest sharing.
Invite students to offer positive feedback or helpful suggestions for classmates. This builds a supportive classroom community where everyone feels motivated.
Summarize key takeaways and help students set mini-goals for the next check-in. Actionable steps keep students moving forward with their SMART goals.
A SMART goals worksheet activity guides students to set and track goals that are Specific, Measurable, Actionable, Realistic, and Time-bound. It helps students visualize, write, and revisit their goals, encouraging ongoing progress and reflection.
Teachers can assign the SMART goals worksheet for students to define personal or academic goals, monitor progress over time, and adjust their plans as needed. The worksheet can be customized for different classes and printed or used digitally for easy tracking.
SMART goals help middle and high school students focus on clear objectives, develop self-management skills, and learn how to break big ambitions into achievable steps, fostering a growth mindset and resilience when plans change.
A strong SMART goal example should be Specific (clearly defined), Measurable (can track progress), Actionable (steps to achieve), Realistic (achievable), and Time-bound (with a deadline). For instance: "Raise my math grade from 80% to 90% by the end of the semester by completing all assignments and attending extra help sessions."
Students should revisit their SMART goals worksheet every month or every few months to check progress, make adjustments, and stay motivated. Regular check-ins help maintain focus and adapt strategies as needed.
