  • What's wrong with this picture? Holly has her things in the middle of the hallway for that Brian can trip on. Often people in the workplace do not keep their belongings in a safe space, out of the reach of other employees
  • The result of Holly's mistake was Brian getting injured and breaking his arm. This isn't just a one time occurrence. According to the National Safety Council 2016 study, they found that 48,060 people had to miss days of work because of an injury at work.
  • What's wrong with this picture?Kelly has knocked over her cup of water into the walk way, causing Rachel to slip and drop the big box of important documents in the water. Now Rachel is injured and important paperwork is ruined.
  • The result of Kelly's mistake was that Rachel twisted her fractured her ankle and had to wear a boot. It may have been something so trivial like accidentally spilling water but it affected Rachel drastically. We must use closed containers in the workplace and if we do spill something, clean it immediately and place a wet floor sign.
  • What's wrong with this picture?A lot of fatal work accidents come from falling from places like stairs and ladders. In Tony's case, he could've avoided this by asking for help instead of taking a bunch of heavy boxes down himself.
  • The result of Tony's mistake was that now he is in the hospital for multiple broken bones from falling off the stairs. He also made the mistake of bringing no one to help him or call 911 immediately after the injury.
