My friend just fell down, I don't know why. This needle fell down with him
No, you won't. According to the second Good Samaritan Law, you will not get in trouble with the authorities if you are the one to call 911 and stay with the victim. 
Unfortunately, it seems as if your friend overdosed on some sort of drug. Please call the emergency services
Oh, no! Am I going to get in trouble over being involved with drugs??
According to my sweep of my surroundings, there are no other hazards to me and the victim. I will start checking him for injuries
I have determined that he is unconscious, I will start my tap-shout-tap method...Are you okay!?...
I have called 9-1-1, EMTs are on the way.
As the victim is still unresponsive, I have implied consent to give first aid
