- MaryAnn was a distracted worker.
- Hi, I'm MaryAnn, I'm 21, I work at a salon and I slipped on a puddle of product that was not properly cleaned up because I was not looking where I was going. I fractured my rib and had to be off for six weeks. My supervisor told me that I should have been more alert.
- Jared was not careful with chemical hazards.
- Hi, I'm Jared, I'm 19 and I work at the movie theatre. One day, I was cleaning the bathrooms and i went to grab a drink. I came back with my drink and I put it beside the toilet cleaner. I reached to grab my drink but I accidentally drank the cleaner instead. I should have waited to have my break instead of taking a beverage into the washrooms with me.

- Hi, I'm Lyla, I'm 17 and I work in the stock room at a grocery store. I was going to use the forklift to pick up a very heavy box, but my co-worker told me to just pick it up with my arms. I bent down and picked up the box but i dropped it on my leg and I toppled backward. I should have told my co-worker that I felt unsafe picking up the box.

- Hi, my name is Dave, I'm 22 and I work on a construction site. I was working below someone hammering up some boards. I wasn't wearing my hard hat and I was listening to music so I didn't hear my co-worker say "Heads!" The wood was dropped and landed directly on my head. I was in the hospital for weeks.

- Hi, my name is Sasha, I'm 19. I work at a library and I had to get a book from the highest shelf. My training did not go over ladder safety and I went on the top step of the ladder. It was too high so I lost my balance and fell off of the ladder.
- Dean's ergonomic requirements were not met.
- Hi, my name is Dean, I'm 21 and I work as a tutor. I was not provided with a chair and I work 8 hours a day with not many breaks. I stand hours on end, which is really bad fore my back. Because I have back problems, it alters my sleep schedule and patterns causing me to be very tired and sore all day.
