Not a problem. First check if the provider is listed in the patient consent form, second check the date when the consent was obtained and if it is still valid.

yes, that is very important. According to HIPAA privacy and security rules PHI of a patient may not be sent beyond a specific time frame
Yes I checked if the provider was included in which he was, but I did not pay attention to the expiry date.
Additionally the risk analysis committee can release a PHI of patient without authorization / consent upon specific cases like child abuse, elderly neglect, public health law, if there is a detection of fradulent activity.
