- Not a problem. First check if the provider is listed in the patient consent form, second check the date when the consent was obtained and if it is still valid.

- yes, that is very important. According to HIPAA privacy and security rules PHI of a patient may not be sent beyond a specific time frame
- Yes I checked if the provider was included in which he was, but I did not pay attention to the expiry date.
- Additionally the risk analysis committee can release a PHI of patient without authorization / consent upon specific cases like child abuse, elderly neglect, public health law, if there is a detection of fradulent activity.
