  • The Referral and assessment nurse at the Outpatients Physiotherapy Unit of an Acute trust receives a referral letter from Mr Jonah's GP. She assesses the letter and schedule an initial assessment session with the musculoskeletal physiotherapist Dr. Sam.
  • RECORD UPDATING
  • I'm really happy with Mr Jonah's improvement.
  • The Musculoskeletal physiotherapist, Dr. Sam evaluates Mr. Jonah medical records and physical condition to identify the cause of his severe knee pain. Dr. Sam then prepares a treatment plan for the specialist physiotherapy nurse to follow.
  • BASELINE FINAL ASSESSMENT
  • Mr Jonah, I am really happy with your assessment. How are you feeling?
  • The specialist physiotherapist nurse, Ms. Anna guides Mr. Jonah through a series of exercises weekly to help heal the knee pain. She also gives him advice on how to manage the knee to avoid an escalation of the pain.
  • DISCHARGE
  • Thank you very much!
  • Ms. Anna records Mr Jonah's treatment progress weekly for Dr. Sam's evaluation and updates the treatment plan based on Dr. Sam's recommendations.
  • Every 3 months, Dr Sam meets with Mr Jonah to carry out a baseline assessment and at the end of the treatment period (12 months), a final assessment is done to ascertain the pain has been dealt with.
  • I feel better Doctor. The pain is gone. Ms. Anna has been amazing
  • After the final assessment, Mr Jonah is issued a outpatient letter. This letter is also sent to his GP who referred him to the clinic.
  • Here is your outpatient letter. A copy will also be sent to your GP.
