Tinnitus, Wheelchair User, Long-standing Health Condition, Mental Health Issue, Visual Impairment
I would like to be in my flat where I can have nurses and carers visiting and where I can speak to people and i don't think 2 weeks rehab is long enough
Hi Jerry, I am Chloe, your Discharge Co-ordinatorI agree with the Dr, it's probably best as it's free. You'll receive a couple of weeks rehab support.
Week 10 in Basildon Hospital
The staff have been great here, I really want to go back to my flat now.
You're now medically well enough to be discharged. You can go to a care home or there are other options. Perhaps it would be best for you as the service is free for 4-6 weeks.
1st Care Home
Hi Jerry, do you remember me? I am Sharon, your Physio.Have you been following your exercise plan ok?
Hi Matt, I am moving care homes as I am having such a bad experience here. I've only seen management once. No one is helping me follow my plan. I feel scared due to numbness and stiffness in my legs.
I don't know what my rights are and what I can and can't ask for. It takes me ages to open the letters they give me.
121 days after being discharged from hospital
2nd Care Home
Hi Jerry, do you remember me? I am Rio your Nurse. How is everything going?
I need a specialist wheelchair and support with accessing a ground floor flat and I can go home.I can't access outside, have a nice bath and I've only been out of bed for a total of 3 days.
Good to see you Rio. I'm not great. . I have no choice on anything, only food. I need a customised wheel chair and I can't move around or go home.I do have support from AGE UK peer support and also the mental health community team who want to help me get back to independence. The only person I really speak to in here is you. I want to be independent so badly.
