- Good morning sir, I am happy to help. You will need to fill out either a written or electronic authorization form that confirms your demographic information and nature of the request.
- Not a problem, here is my driver's license. I just completed the electronic authorization form. What happens next regarding my request?
- Thank you Mr. Smith for providing your driver's license. Once the electronic authorization is reviewed, our facility will confirm the identity and legitimacy of the new clinician and facility before sending only the relevant details of the request from your medical records.

- Nice response Sam, I couldn't have said it better myself. The only thing I would add is that we utilize a secure, encrypted network when transferring the patient information which ensures confidentiality of the electronic health information.
- I appreciate the constructive feedback sir. Now that you mention it, I understand why explaining that information to the patient would be relevant and make them feel more comfortable with us sending their heath information.
