- Hi Teri and Dennis, you wanted to see me?
- Hi Susan. Yes, Dennis and I wanted to go over a Med Event we were reviewing. We wanted your input before sharing with the team.
- OK, what is the Med Event?
- The prescription is for a twin sister with a similar name as her brother.
- Yeah, the first names are similar. The prescription is for twin sister Thi Anna but filled for her brother, Thai D.
- The twins are listed on the same account with similar names and matching DOBs. So, we can see how the incorrect patient was selected. However, the names are similar and would have been bolded and highlighted in different colors in F14. Extra care should have been taken when selecting the patient.
- What is the prescription?
- The prescription is for oral contraceptives, which is why we wanted to discuss it with you. We do not know the clinical impact of sending it to the brother.
- Well, there's not enough hormone to change the brother's hormone balance if a couple of pills are taken. However, if taken over an extended period of time it may cause infertility, decrease sex drive, decrease muscle mass and osteoporosis.Other side effects could include headache, nausea, muscle pain, depression and fatigue.
- Wow, we really need to let the techs know about the clinical impact when the incorrect medication is sent to the wrong patient.
