  • So my routine day comes to an end! All equipment is ready for service! Time to head back to my room and relax!
  • I just fell asleep, whats the issue?
  • There has been a leakage, we are turning the plant down, come quickly.
  • Why did you not prep the equipment?.........I dont wanna hear it now!
  • Sir! This proves we informed them about the spare equipment and are not responsible!
  • ......But sir we did.....

  • This is the plant maintenance engineer, returning from his usual shift after addressing all the equipment issues and ensuring availability if required.
  • This might prove our innocence but can lead to huge future problems for us if ops is pitched against us!
  • I think we all need to do our part since we cannot identify 1 single cause.
  • He gets a call from the shift engineer regarding the emergency related to HP Ammonia Pump. The plant is shut down before he arrives instead of taking the spare equipment into service.

  • Since we can't identify a direct cause, everyone will have to take corrective action!

  • The manager blames the maintenance engineer for not prepping the equipment while the workers work on the equipment, snubbing any reply. Afterwards, the engineer shares evidence which spares maintenance from the blame with the maintenance head.

  • The maintenance head decides not to use the evidence that may lead to resentment within ops. The investigator is tackled by giving generic feedback on the incident. His interest is served as ops department is not targeted specifically and his relationship with the ops is maintained.

  • The outcome of investigation suggests that since the incident cannot be traced back to one cause, all departments will collectively take responsibility and take corrective and preventive action. This comes as a surprise to the maintenance engineer.
  • The shift engineer is rewarded for his reaction to the incident. Since the communcication gap and operational philosophy is not highlighted during the investigation, he is hailed as a hero for preventing huge scale damage by shutting the plant down.
