John is 62 years old admitted in hospital in urgent care unit due to shortness of breath, low saturations, persistent cough ,chest pain and fever
Past medical history; Lung cancer, COPD, sever emphysema, Bronchiectasis and AF/flutter
Plan:Blood test, ABG, ECG, Nebs, CXR, Centime PRN, Chest drain, monitor oxygen level, and analgesia. Pleural nurse specialist R/V, ENT R/V and ICU Register R/V
Diagnosis: Pneumothorax
Pneumothorax is when air gets into the space between the outside of your lung and the inside of your chest wall and ribcage. (MacDuff, Arnold Harvey,2021)
The Nurse informed John about the treatment plan and checked regular observations. Nurse liaises with multidisciplinary team(MDT) regarding treatment plan. John had CT done, blood test, ABG and ECG. Pleural nurse specialist saw the patient. ENT report came back normal showing that the upper airway is clean. CT shows large Pneumothorax. Dr informed John he will need a chest drain. He then explained about Thro-vent drain for pneumothorax and procedure.
I have taken John blood pressure, temperature, pulse with nurse supervision. John was struggling with breathing and high news score. The Nurse administered oxygen to improve John's breathing and escalated it to the nurse in charge, due to the high news score. Then the Nurse informed Dr's to review patient.
