  • Nurse had a discussion with the MDT. Medical Dr inserted Thora-vent drain device on John chest to remove excess air in the lung. John breathing has improved, as John reported he can breath much better soon after excess air has been removed. John still has shortness of breath when he walks to the toilet and back. The IV antibiotic seems to responding well John and regular analgesia.
  • Pleural nurse assessed John and explained how to look after the Thora-vent drain and what support John may need if he will be discharged with device home. She also explained to the nurse how to monitor/ aspirate . He can be discharged from the hospital with a Thora-Vent if it lasts 7 days or more. However, John require home oxygen. patient-centered health care quality and safety goals to achieve care that is safe, effective, patient-centered, timely, efficient and equitable for John.
  •  Thora-Vent (also called a thoracic vent) is a small device that’s connected to John chest. It has a one-way valve with a self-sealing port and a small catheter (thin, flexible tube) that lets extra air out of John chest. The goal is to let it fully expand the left lung (Gao and Gao,2021) . 
  • Nurse informed John he will be moved respiratory ward. Nurse prepared John, she filled out the transfer sheet and handover to the nurse who will be looking after John.
  •  The role of the nurse is to advocate for the best interests of the patient care and to maintain the patient’s dignity throughout treatment and care. (NMC Future Nurse,2018)
