- No problem. Lets start by getting that wound on your arm looked at and get a nice new dressing put on!I'll update your wound record here on my tablet device once we are done.
- Hi Jane, welcome home! I will be your District Nurse, helping you recover over the next few months here in your own home.
- Oh thank you so much! I am just so glad to be home!

- Ok that new dressing is all done. I've updated your wound record on my tablet device and I can see the wound it is healing very well. The size has decreased by 5cm since the last wound record which was entered in the nursing home.Did you have anything else I can help you with before I go?
- Ok that's no good, I will ask you some questions and fill in this pain assessment on my tablet device.
- I have been struggling to sleep, this arm has been hurting and aching so much in the night.

- Ok thats the pain assessment all done. I can see from your previous pain assessments in the hospital and nursing home that the pain has been severe but has been improving. I'll speak to your GP and see if we can get you some medication to help you with the pain whilst you sleep. Good quality sleep is important for your recovery. Goodnight, I'll see you in the morning!

- Sorry about that Jane. There had been an error with your medication. I am certainly glad we checked!You should not have been prescribed Ibuprofen. I can see in MAR record you were very poorly last time you took ibuprofen at the nursing home.The doctor has prescribed you this Paracetamol instead.
